* Denotes a required field Terms of Payment: Payments made using this online system will be assessed a convenience fee of 2.39% ($1.50 minimum per transaction for amounts less than $60). The fee for your payment will be noted on the checkout page before you complete your transaction. Please note that ALL FEES ARE NON-REFUNDABLE. Payments made will be receipted to the next business day. For example, if your license/permit/special event is on the first of the month, you need to make the payment before midnight Arizona time on the previous business day. If the first of the month falls on a weekend or holiday, you need to make the payment the previous business day before midnight Arizona time. Example, if you have a payment due March 1st, you need to make the payment on February 27th before 5:00 p.m. and if February 27th falls on a weekend, you need to make it on Friday, February 25th or 26th. Special Permit applications must be received by the Environmental Health offices prior to or at the time of payment. The Company/Organization must be the same as the name on the application.
